order fill coming from nowhere

Hello, an order has been placed last night and I do not know where it comes from (no manual intervention or automatic trading). I have post the trade log. Can you help me to understand in order to prevent this kind of problem ? It is the third trade of the log.

thank you for the answer.
I just realized what has happened. During the day an buy order was placed, but the platform did not see the order was filled, and then tried to cancel this order, and finaly did not see the position. But on the broker side, the order has been filled. What I see at the end of the day is just the correction of the position. I have attached both logs.

My question is how to detect or prevent this behaviour ?

This is not clear for us. We do not know specifically what the issue is. You are going to understand this best.

And we have no control over the trading service you are using.

We see that there was a nonresponsive order. That should just not happen unless there is a connectivity issue.
