Standing OCO Orders Apparently Canceled and Position Flattened

I've seen this happen before. I had a OCO order on ES minis. Was short from 2964.5. Stop either at 2968 OR 1 off at 2961.25 and move remaining order to break-even.

I turned to my other platforms and hear the chime of the execution. Position was flattened at 64.5, break even. It remained below this level and continued down into 2959 or so.

I use this order style all the time and it usually behaves as expected, however, like I said, I've seen this happen before.

How can I debug what's going on here?

Really it is not completely clear what you are describing but you can have a look at the Trade Activity Log to analyze the trading activity:
