Duplicate orders created with a single click when using IB

Dear Support,

I use Sierra with Interactive Brokers (advisor account, SC realtime data displayed, but orders sent to IB to multiple accounts, 'Use Order Fill Calculated Positions' option enabled).
Until now I never had any issue with execution or placing orders.

Now I am randomly encountering with the following problem:

When I place a resting order (limit or stop), there are duplicate orders created.
For example I click on the DOM to place a buy limit order of 2 contracts and 2x2 buy limit orders appear - two separate orders of 2 contracts each.
This happens not just when I place orders with manual clicks on DOM, but when I use trade associated hotkeys, like cancel and flatten all (pending orders are cancelled correctly but duplicate exit order appears).

Unfortunately this problem comes up randomly, although I can provide video record of it.
Can you please advise how can I investigate this problem further or if there is a fix for this?

I am using the exact same setup since many months and never experienced an issue like this (overally I had zero issues with the IB/SC setup - everything works perfectly).

First thing we need you to do is to follow through the instructions here:
Trade Activity Log Analysis: Support Assisted Trade Activity Log Analysis

We need to know the Internal Order ID of one of these orders and we will see what the source is of the second order.

There is no need to pay anything extra for this.

We see no evidence in the Trade Activity Log of a duplicate order at that particular time. Look at the Trade Activity Log following these instructions:
Trade Activity Log: Viewing Historical Trade Activity

And let us know where exactly you see that duplicate order.

I have checked it based on the instructions and I do not see the duplicate order either

Although please see the attached screen record - unfortunately I can not describe/show it more precisely:

The selected trade quantity is 2, but when I click on the DOM to place a buy limit order, there are 2 buy limit orders placed: 2 contracts each.

Until now seemingly this happens randomly. Tried to check if it is a hardware issue (mouse) but the same happens with another mouse and even when using trading hotkeys ('flatten and cancel' key sometimes sends two identical closing order).

Can you give any advice about how to investigate it further?

Okay we see the video, but those orders must show up in the Trade Activity Log. Just tell us the date and time you submitted one of those orders that is a duplicate and the time zone and then attach the Trade Activity Log again. But before you attach it, exit from Sierra Chart so the cached is flushed.

Please see the Trade Activity Log attached again.

The date time when this happened:

2018-09-24 from 10:43:51 - New York Time Zone (both SC and system clock is set to this timezone)

Video timestamp is the same (2018-09-24 10:43)

Based on the video there must be two orders but for reasons unknown we do not see those two orders in the Trade Activity Log which has no explanation at all from our side. This really is very puzzling. When you see those two orders, go to the Trade >> Trade Orders and Positions >> Orders tab. Select those two orders and select Edit >> Copy Selection and paste them here.
