I have not traded live in several months. Today I switched to the live account and 3 times I entered a bracket order. I could see all three elements on my chart (the pending entry, stop and target). Each time, as soon as price reached my selected entry level, the order was filled and immediately the target order disappeared. There was an error message saying a modification could not occur to the same price. Any suggestions for correction?

What are the settings for your Entry and attached orders? Getting us an image of the Main tab and Attached orders tab of the Trade Window you have setup for this would be easiest.

Also, we need to understand how you are entering the main order in the first place. Are you using the Trade Window, or are you entering from the DOM or the chart or something else? And what is the order type that you are entering with?

Attempt 2 to send. Not sure if my previous attemt went through so I cleaned up the chart further and am trying again.
I entered the bracket order using the dom on the right side of chart. I clicked on the price I wanted to enter in the green 'buy' column. After the price activated the market order the target disappeared. It does not behave this way when I am in Sim mode.

The reason for this is that you are using a "Market if Touched" for your Target. This kind of order is not allowed by the CME Group exchanges. So that order is being rejected, in which case the Stop is then also removed since it is an OCO pair.

The order type of Market if Touched does work in Sim, as we do not block it, as some exchanges will accept it.
