This is the reason for the connection problem:

CQG FIX: Received a Logout message. Text = Username or password is incorrect. | 2016-05-18 17:41:12
CQG FIX: Sending a Logout message. Text = Received Logout message. | 2016-05-18 17:41:12

Select Global Settings >> Data/Trade Service Settings. Make sure Connection Details >> Username and Password are set correctly. You need to contact your broker for those.
