The log is very clear. One way or another the Flatten and Cancel button on a Trade Window was pressed. Perhaps even just selecting that button and pressing the space bar can trigger it. This command can even be added to the chart shortcut menu which displays when you right-click on a chart. And there is a keyboard command which can also invoke Flatten if it is configured through Global Settings >> Customize Keyboard Shortcuts.
