I am just using macro express because it's simple and I have a mouse left click on the top of the window where my keyboard trading shortcuts are active. For me, that's a Trading DOM as I like to have things in separate instances. It has been 100% reliable but there is one last thing I did to make it reliable. I have tied the macro's keyboard shortcut to a mouse button my Logitech MX 2S. Before that it was not 100% reliable but now it is. If you are manually clicking on the trading window then there should be zero issues as it is then clearly in focus and the only explanation I can think of in that situation is that some other window is bringing itself back into focus before you execute your trade
