I submitted a live order today. On the Trade Window that was attached to my chart the Use Attached Orders box was checked.

On the Targets tab I have Target 1 Offset Type set to Ticks, Type set to Target:Limit and Limit Offset on 2. I also filled out the Stop as Type Stop:Stop and Stop Offset for 35 ticks.

I sold short NQU21_FUT_CME at the market. The fill was 15594.50, then the trade was closed with a buy order at 15594.50

I also have Global Settings-General Trade Settings-Adjust Attached Orders to Maintain Same Offset on Parent Fill set to Yes.

According to the documentation, "when the parent order fills, these offsets are then always relative to the fill price...."

Why then was the trade closed out for the exact same price as the parent order was executed instead of 2 ticks lower as instructed?

In the case of the Sierra Chart Order Routing Service, this functionality is managed on the server instead of within Sierra Chart:
Global Settings-General Trade Settings-Adjust Attached Orders to Maintain Same Offset on Parent Fill set to Yes.

We determined yesterday, that this functionality is not functioning on the server, due to a related feature request implemented for a another user which caused an issue with this functionality. The issue has been resolved and tested and we will be releasing the solution for this, at the end of the day today.
