Attached order not getting submitted

I put in a limit order with two attached orders - Limit and stop limit with "Move to breakeven stop" as "Good Till Cancelled" . I can see my limit and both OCO orders on the chart before the fill however, once my limit order is filled both the OCO orders disappear.
Last night, I left my system ON and SC client connected but it still happened.
The issue doesn't happen when I am using SIM account, FYI.
