Signal order executed without price reaching target

I have an automated trading program and on Sunday I had an execution of an order without the last price meeting the condition. To explain further I have an indicator that if the last price go below it sells, the order executed 4 handles/16 ticks above! 2 hours after the market open...any ideas of why will this happen?
