Chartbook fails to appear in Recent File List

This is weird and hard to explain. The attached chartbook is very simple, but when I open it (SC 1544) it does NOT appear in the Recent File list.
Steps to reproduce on my machine:
1. File + Clear Recent File List
2. File + Open Chartbook + select Debug2
At this point I do NOT see Debug2.cht in the recent file list. It is empty.
3. File + Close Chartbook
At this point I do NOT see Debug2.cht in the recent file list. It is empty. (But, if I would select the empty 1 line in the recent file list, Debug2.cht would open.)
4. File + Open Chartbook + select any other chartbook, e.g. Tmp1.cht
At this point the recent file list is:
1 Tmp1.cht
2 Debug2.cht
5. On Tmp1.cht, do File + Close Chartbook
On Debug2.cht, do File + Close Chartbook
At this point the recent file list is the same as above.
6. File + select Tmp1.cht from the recent file list
Debug2.cht opens, NOT Tmp1.cht

This does not happen with most chartbooks. So far it only happens on Debug2.cht and a Debug.cht from which I did Save As and deleted a bunch of studies. So 2 problem chartbooks out of 60+.

I have seen this happen on two different instances/folders. I didn't see this behavior until recently, maybe 1540 or thereafter

This problem related to some abnormal behavior related to the menus managed by the operating system.

We implemented a workaround.
