Bracket order failed to execute

I recently had a position opened with a bracket order and the stop failed execute. what would cause that? Im assuming that the stops and targets for brackets are sent to the respective accounts and not held locally is this correct?

Im using Rithmic currently and it happened again but this time only with a bracket order i placed an order and my stop was hit but the limit wasnt cancelled. it appears my target gets placed using a held status? i had to manually cancel that after the stop was hit.

How do you have this particular setting set?:
Global Trade Settings Windows: Use Server Side OCO Orders (if supported) (Global Settings >> General Trade Settings >> General)

Both are checked I read in one of the forums this is the solution to when the server rejects an order.

We did identify a problem when both of those options are enabled and it will be resolved with the new release coming out later today.
