Serious DOM issues (orders dissapearing)

Have the following two issues:

1) orders disappearing from the DOM after entry executed (e.g. I enter long CL at 40.20 and 1 second after entry DOM deletes the order); but order still there. Renders DOMs useless and dangerous. Happening now, all the time.

2) "cancell all" button seemingly cancelling resting orders, but not from the broker. Happened once. Just had a stop order I had supposedly cancelled execute - found out thanks to my broker (email).

Please help.

1. You have received notices about this when starting Sierra Chart in the System Notification Window. Here it is:
Notice: Important Update for Interactive Brokers Users

Do not disregard those notices. Older versions of Sierra Chart will be locked out for Interactive Brokers users in about two weeks.

2. This does not make sense. You are saying that Cancel All is seemingly canceling orders but then you are saying it is not. In any case, trading activity is logged in the Trade Activity Log:

2) Thought was clear enough

"cancell all" button eliminated the order visually from the DOM

but did not eliminate the order from my broker

2. If you look at the Trade Activity Log for that particular order you should see a line that puts the order status to "Pending Modify" for that order. At that point in time, the order was canceled through the TWS API.

So you should see that line confirming this.
