No solution here. But I do agree with point #1... the Chart Trade Menu should be stacked (or an option to be stacked), not side-by-side with non-trade commands. I too, at times have placed trades by mistake because of the placement of trade commands on the right-click. Maybe a right-click+somekey or better a middle button click, to bring up ONLY the trade menu.
