The problem is with Trader Workstation. If you can avoid using Interactive Brokers that would be best. You can see for yourself just by having a look at
the Trade Positions data received from Interactive Brokers in Trade >> Trade Orders and Positions >> Positions.

We have other reports of this exact same problem. And has been thoroughly looked into yesterday, and determined to be a problem with Trader Workstation.

Regarding this:
but when the target or stop price is hit a REVERSE trade is automatically initiated.

We need to see the lines in the Trade Activity Log for the order which is reversing the Position to see what initiated it. Here are instructions:
