Unfortunately we can still not reproduce this. So we just have to think about this some more. We are not really quite sure what the issue is. Maybe a Message Log would help us.

Follow the instructions here to update the Symbol Settings:
Global Symbol Settings: Update Global Symbol Settings

Also disable the use of Custom Symbol Settings to see if that is causing some unexpected issue.
