I've also copied a working instance, renamed it to the broken instance and copied these config files over from the broken instance to see if I could find where the problem was:

Accounts.config
DataFilesFolder.txt
KeyboardShortcuts.config
Sierra3.cfg
Username.txt
VersionNumber.txt
MenuCustomizations.xml
AccountBalance.data
lnternal0rderlD2.data
TradeOrdersList.data
TradePositions.data

This was still causing the windows dialogue error when starting in safe mode. So I copied back the working Sierra3.cfg file from the original working Instance and it's now working again.

So looks like the Sierra3.cfg file is causing the issue.

So now I reinstated the broken Instance using a backup of the Sierra3.cfg and am back on 2109
