First "problem" is that connecting Sierra to TWS I have to confirm by Yes twice.
This is a TWS issue. There is nothing we can do from our side in regards to this. It is best that you set 127.0.0.1 as a trusted IP. Refer to the Interactive Brokers set up instructions about this:

Make sure you are not using the order fill calculated position. It sounds like you are. For further information, refer to:
